What is the Immigration Medical Exam?
The USCIS (United States Citizenship and Immigration Services) medical examination is a crucial step in the immigration process for individuals seeking to enter the United States or adjust their immigration status. During the USCIS medical examination, you will undergo a thorough medical evaluation which includes a physical examination, as well as a review of your medical history and vaccination record. The results of this examination are documented on the USCIS Form I-693.
Does a civil surgeon conduct the exam?
Yes, the examination is conducted by Dr. Diamond, an authorized civil surgeon.
What are all the costs?
• Medical exam: $400
• Labs: $140
The required labs are Quantiferon, RPR, and Neisseria gonorrhea.
• Vaccines: TBD
Most vaccines are offered at our clinic, but you are not required to get them here. You can obtain the necessary vaccines either from us or at another clinic or pharmacy.

What am I required to bring to my appointment?
1. Your I-693 Form
Your portion needs to be completed BEFORE the appointment. This includes: name, address, country of birth on the first page; phone number and email address on the second page. Please put your name on the top of every page. PLEASE DO NOT SIGN IT UNTIL YOUR APPOINTMENT!
2. Your Driver’s License or Passport
3. Your vaccine record
This includes COVID vaccination and all childhood and/or adult vaccinations. If you do not have a vaccine record, you will have to get all vaccines that are required for your age at a local pharmacy. You are required to have the 2023-2024 Covid Vaccination.
4. Prior medical records
If you have an extensive health history or have been treated for tuberculosis in the past, bring your records.
5. Translator
If you do not speak English. (This can be a friend or family member who speaks both English and your native language).
What can I expect during the exam?
Dr. Diamond will conduct a comprehensive evaluation to assess your overall health and determine if any medical conditions might impact your admissibility. The examination typically involves the following components:
1. Medical History Review:
Dr Diamond will discuss your medical history, including previous illnesses, surgeries, and chronic conditions.
2. Physical Examination:
A thorough physical examination will be performed to evaluate your general health.
3. Laboratory Tests:
Depending on your age and medical history, Dr. Diamond may order specific laboratory tests such as blood tests to screen for tuberculosis, syphilis, and gonorrhea.
4. Vaccination Review:
Dr Diamond will review your vaccination history and ensure that you have received all the required vaccinations according to the U.S. vaccination guidelines. If any vaccinations are missing, you will go to a local pharmacy to have these administered.
What is the whole process?
Visit 1
Physical exam, receive lab order, review vaccine requirements
After visit 1
Take your lab order to a Lab Corp to perform tests.
Complete any outstanding vaccines.
Visit 2 (approximately 7 days after labs)
Pick up completed paperwork. You will receive the sealed original I-693 Form and a personal copy of I-693 Form. Most people will NOT have to see the doctor at this visit.

Adult 65+
• Tdap - 1-time dose of Tdap for Td booster; then boost with Td or Tdap every 10 years
• Polio IPV/OPV
• Flu - (if it is flu season)
• Covid 2024 - 2025
• PCV15 (one) followed by PPSV23 or one dose PCV20
• Varicella - 2 doses or proof of immunity, or stated history of chicken pox
Adult 18-64
• Tdap - 1-time dose of Tdap for Td booster; then boost with Td or Tdap every 10 years
• Polio IPV/OPV
• Flu - (if it is flu season)
• Covid 2024 - 2025
• Varicella - 2 doses or proof of immunity, or stated history of chicken pox
